    Before we can officially offer you a position with us, we must first get all the required information/documentation and agreements from you so we can confirm this is the right fit for us, you and our clients.

    We need you to complete this form and submit it back to us at your earliest convenience.

    We ask that you have all your documents scanned and saved on your computer before you begin. We suggest you use the CHROME Browser to complete this induction.

    The documents that we require you to have available are:

    • 100 Points of ID and Proof you have working rights in Australia. 
    • Drivers Licence (a clear copy of front and back)
    • NDIS Workers Screening Check. This is a new NDIS requirement that includes the Police check and allows you to have your fee waived for the WWCC employee. Apply here.
    • Working With Children Check (WWCC)- Employee - Apply here
    • A completed Tax File Declaration 
    • A completed Superannuation choice form
    • Valid First Aid Certificate (less than 3-years-old)
    • CPR Certificate (Less than 1-year-old)
    • Copies of any Degrees, Certificates, proof of participation etc you think may be relevant to this position
    • Evidence of Vehicle Insurance
    • A portrait picture of yourself
    • A certificate of completion of the NDIS worker Orientation module can be obtained here and following the step-by-step guide. We suggest you complete this step before beginning this form.

    Please note you are responsible for providing us with your Superannuation details (Super fund and member number etc). Unfortunately, you will not be able to choose Employer nominated, you either need to create a new superannuation fund with one of your choice, that is APRA or Self-Managed or provide the details of an existing Superannuation you have in your name."

    Caring Lotus Resources Pty Ltd pays wages weekly through direct deposit. We will require your full bank account details to pay wages into your account

  • Important NDIS Screening is required for this role

    All employees performing risk-assessed roles are legally required to hold a valid NDIS Worker Screening Check when working with an NDIS-registered provider. As you will be delivering services on behalf of Caring Lotus Support Services Pty Ltd, a registered NDIS provider, this requirement applies to you.

    The NDIS Worker Screening check will last for 5 years. Workers can apply for the NDIS Working screening check at the following location https://www.service.vic.gov.au/services/national-disability-insurance-scheme.

  • Current Working With Children Card (WWCC) - EMPLOYEE

    You will be employed under Caring Lotus Resources Pty Ltd if you are a support worker and Caring Lotus Pty Ltd if you are working in the office. We require all workers to have a current Employee Working With Children's Card (not a volunteer one) regardless if they are working directly with children or not. If you don't have an employee working with children card, you will need to submit evidence you have applied for this during the orientation phase.

    Apply for the Working with children check here. If you don't have a card, you can apply for the NDIS screening check first, and once this is approved, you can apply to waive your WWCC fee by using this link.

    If you hold a volunteer card, you can update this to an employee one by going here.

  • Workers Orientation Module

    NDIS requires that all workers complete the Workers Orientation Module 'Quality, Safety and you'.

    If you don't have a certificate already, you will need to create an account if you don't have one yet. It can take up to about 90 minutes to complete. 

    Once complete, save the certificate as you will have to upload it if you are successful with employment. 

  • You MUST complete your NDIS Workers Orientation module to work with us.

    We will not make you compliant or able to be rostered onto shifts prior to completing the NDIS Workers orientation Module.
