    Client is agreeing that they have the knowledge and understanding of Caring Lotus's processes and Procedures - Available through our client resources on our website

    This form covers our Risk assessment and emergency plan. Once completed you will receive an email with a PDF of the client's Emergency plan that should be printed and placed with their intake pack so they can put it up in their home.

  • HOW TO COMPLETE THE INDIVIDUAL RISK PROFILE

    All steps must be completed

    Individual Risk Profile- Risk Checklist (IRP1) - Support Planner

    1. Review column 1(Risk Areas) to identify actual & potential risks.
    2. Place an X in the relevant box(es) in columns 2, 3, 4 as to whether the risk is to participant, staff or others.
    3. Use the “Risk Indicator Chart” on the right to assess and select the risk category – Extreme, High, Moderate or Low
    4. In column 5 (Source of Information), write down how the risk was identified.
    5. In column 6 (Current management plans/ guides) write down the participant’s current plans or guides to control the risk.

    Individual Risk Profile- Risk Management Plan (IRP2) - Support Planner

    1. An entry is made for every identified risk.
    2. Column 1- place a colour dot/tab (red, orange, yellow, green) to indicate level of risk (refer to IRP1).
    3. Refer to IRP instructions for completion Columns 2 to 8.
    4. Support Planner to sign and date IRP Sign Off Page.
    5. Line Manager & Senior Manager to endorse IRP1 and 2 by signing and dating IRP Sign Off Page.
    6. Each time a participant’s Individual Risk Profile is reviewed (quarterly) the Support Planner and Line Manager must sign and date.

    • Individual risk profile is a guide only. Any additional risk can be recorded under “Other”.
    • Complete all relevant areas - a risk can be to participant plus staff plus others.
    • Refer to Risk Indicator Chart on previous page to assess risk category (Extreme = red, High = orange, Moderate = yellow, Low = green).
    • Source of information refers to how risk was identified e.g. observation, report from others, Incident Report Form (previous GPS/IRF), workers compensation record etc.
    • Current management plans refers to written guide in place to treat the risk, including behaviour management plans, health care plans, lifestyle and environment support needs recorded in the Participant’s Support Plan.

  • Risk Management Plan

    • The Participant Risk Management Plan describes treatments/actions in place or required to manage risks identified above.
    • The participant and staff who work with the participant must be consulted; others should be consulted, as appropriate. 
    • The intake compliance officer completes the form.
    • All steps must be completed - a new entry is made for every risk identified in IRP1.

    Instructions for each column

    1. Risk Category - indicate the level of risk being red, orange, yellow or green.
    2. Risk - the actual/potential risk – refer to previous answers.
    3. Conditions - relevant situations, triggers, indicators that might result in the risk behaviour – e.g. dust can lead to asthma, having to wait can lead to self-injuries behaviour or verbal or physical assault of others.
    4. Person Responsible - who is to develop/implement management plan/guide/treatment action – Support Planner/Line Manager /Staff.
    5. Due Date - the date that plan/guide/treatment action is due for development.
    6. Date Implemented & reviewed - dates for implementation/review of plan/guide/treatment action.
    7. Risk after control - identify the new risk rating after the implementation of controls.
