Before we can officially offer you a position with us, we must first get all the required information/documentation and agreements from you so we can confirm this is the right fit for us, you and our clients.
We need you to complete this form and submit it back to us at your earliest convenience.
We ask that you have all your documents scanned and saved on your computer before you begin. We suggest you use the CHROME Browser to complete this induction.
The documents that we require you to have available are:
- 100 Points of ID and Proof you have working rights in Australia.
- Drivers Licence (a clear copy of front and back)
- NDIS Workers Screening Check. This is a new NDIS requirement that includes the Police check and allows you to have your fee waived for the WWCC employee. Apply here.
- Working With Children Check (WWCC)- Employee - Apply here
- A completed Tax File Declaration
- A completed Superannuation choice form
- Valid First Aid Certificate (less than 3-years-old)
- CPR Certificate (Less than 1-year-old)
- Copies of any Degrees, Certificates, proof of participation etc you think may be relevant to this position
- Comprehensive Vehicle Insurance evidence
- A portrait picture of yourself
- A certificate of completion of the NDIS worker Orientation module can be obtained here and following the step-by-step guide. We suggest you complete this step before beginning this form.
